New Patient Registration

We welcome new patients within our practice area. All patients wishing to register must collect a registration pack from reception. We will ask you to provide photographic proof of identity, e.g. passport, driving licence, etc. and written proof of address, e.g. utility bill, bank statement or tenancy agreement.

In the registration pack there is a short questionnaire to help us obtain an immediate background to your medical history. Please complete this information and return it to reception with your completed registration form.

The practice boundary includes NE1, NE2, NE4, NE5 and NE15 but there are restrictions within these. Reception will be able to confirm if your address falls within our ‘catchment’ area.

You must keep the practice informed of up to date telephone numbers and addresses.  We may need to contact you.

If you move whilst registered with the practice, and it is outside our 'catchment area', you will be asked to register with another GP in that area.
