Covid vaccine passport


If you don’t want your identifiable patient data to be shared for purposes except for your own care, you can opt-out by registering a Type 1 Opt-out.

This collection will start on 1 July 2021 so if you do not want your data to be shared with NHS Digital please register your Type 1 Opt-out with the surgery by 23 June 2021.

The data held in your GP medical records is shared with other healthcare professionals for the purposes of your individual care. It is also shared with other organisations to support health and care planning and research.
If you do not want your personally identifiable patient data to be shared outside of your GP practice for purposes except your own care, you can register an opt-out with your GP practice. This is known as a Type 1 Opt-out.

To register a Type 1 Opt-out, for yourself or for a dependent (if you are the parent or legal guardian of the patient) (to Opt-out) you can download the form from the link below and then returning it to the surgery before the deadline of June 23rd 2021

Latest guidance on exemption letters for not wearing face masks

GP surgeries are not obliged to provide letters stating whether patients need to wear a face mask or not.  Please click on the link below for further information.

You will no longer be able to order your repeat prescription by phone from Monday 17 May 2021.  You can order your repeat prescription by using online services (please ask reception if you require access) or by posting your tear off slip through the letterbox at the front door.  We require at least 48 hours notice to process a repeat prescription request.

Coil & Implant clinics

We are able to offer Coil & Implant insertions at the practice once again.  Patients must speak to a doctor before they can be booked in for this service.

If you wish to book an appointment or have any concerns regarding your cervical screening, please telephone the surgery and make a telephone appointment to speak to one of our Practice Nurses.

Coronavirus Updates

Self-referrals to the NHS Volunteer Responders scheme

People who feel they are vulnerable at home during the pandemic, and who would benefit from support from NHS Volunteer Responders, can now self-refer to the scheme.

The number for people to call to make a self-referral is 0808 196 3646.

Unwell Child

Same day help for your poorly child. If your child has become unwell in the last few days and you would like urgent advice then please contact the surgery and we will arrange for a clinician to speak to you and if necessary to see your child, today - this could save you an unnecessary trip to A+E or a Walk-in Centre. Can we also make you aware of the Little Orange Book, which provides expert help for parents and carers on how to manage common problems in babies and small children – ask at reception for a copy.  You can also download a free NHS app which gives helpful advice.
